Call to Care Health Coach at Aspen Hill Health Center, Aspen Hill, MD (also Metro DC area).
General Summary: In collaboration with the team providing clinical care for Aspen Hill Health Center patients enrolled in the Patient Centered Medical Home and other identified Aspen Hill Health Center patients, the health coach is responsible for the development, implementation, coordination and evaluation of tailored patient wellness care plans and health maintenance activities; and the oversight of two peer health coaches. The health coach will ensure patient navigation that includes addressing the social determinants of health through social services, financial assistance and transportation access; as well as providing educational, social and emotional support to patients, families and caregivers to achieve positive health outcomes. The health coach will empower the peer health coaches and patients to become independent, accountable and responsible for their health behaviors. In addition to the development, implementation, coordination and evaluation of a comprehensive wellness and chronic disease (health maintenance) program, the health coach will expertly link participants to county resources such as faith community nurses, community centers and county safety net programs. The health coach will also assist patients with health center appointments and medication access and/or reconciliation. The health coach will also work closely with the wellness coach to prevent hospital admissions and readmissions and increased acuity of illness. The certified health coach will facilitate effective and positive behavioral changes in the areas of physical activity, nutrition, stress management, and chronic condition self-management by using coaching, behavioral and positive psychology, counseling, motivational interviewing, behavioral change theory, appreciative inquiry, adult learning theory, and solution-oriented theory. Overall, the certified health coach will provide program oversight, health education and help each peer coach to work with the clinical staff and patient and their caregivers to develop patient centered prevention, wellness and self-management plans. Health Coach certification may be obtained during the first three months of employment. This is a temporary assignment for the duration of grant funding (over three years). Minimum Requirements: Current American Heart Association Healthcare Provider CPR is required Bachelor’s degree with a major in nursing, nutrition, public health or community health, or social work is required. Master’s Degree in nursing, nutrition and/or public/community health preferred. Health Coach certification is required – prior to or during the first three months of employment. A minimum of two years hospital and health center/clinic experience is required. Managerial and/or supervisory experience required. Linguistic proficiency in a foreign language is preferred. Basic computer skills, to include data entry and retrieval. Reads, writes, speaks, and comprehends English language. Must possess organizational and time management skills. Must be able to effectively communicate by telephone, letter correspondence, fax, and computer. Must have a valid drivers license, automobile insurance, and have access to a car.
